After an initial 1:1 assessment, you’ll be invited to group sessions that cover key aspects of diabetes management. You’ll also receive individualized nutrition follow-up and continuous support from our certified diabetes educators.

Alongside our registered nurses and registered dietitian, you’ll also have access to Morgan McHugh, CRNP, a dedicated nurse practitioner who works closely with our team to support medication management, insulin adjustments, and technology training.

If you’re starting insulin or transitioning to a pump or CGM device, you can schedule an appointment with Morgan for personalized guidance.

Over the course of the program, you’ll develop a personalized plan and education goals to follow, as well as nutrition follow-ups with a registered dietitian. Best of all, you’ll do all of this with group support and community connection!

Yes. DSMES is covered by Medicare and many private insurance plans. Individuals covered by Medicare Part B can participate with no deductible or copay, and the program is often covered by private insurance as well.

A referral from your physician or nurse practitioner may be required, depending on your plan. We’ll help you determine the referral needed for your specific case.
